These instructions are intended to help you prepare your MCSF scholarship application for electronic submission.
Listed below is the information you will be asked to provide, including files to upload, and tips for preparing your essay.

Section I: Identification
– Student ID, Name and School Issued Email address [first.last@stu.fortthomas.kyschools.us] (a copy of the application will be sent to this email)
– Intended institution and field of study
Section II: ACT and Transcript
– ACT Super Scores (Composite, English, Math, Reading and Science)
– Class Rank, Weighted GPA, Unweighted GPA (copied from transcript)
– Upload unofficial school transcript (a PDF file) through fall semester of senior year. [Transcript must match information entered on application.]
Section III: Letter of Recommendation
– Upload PDF file from faculty, staff, coach, or mentor
or
– Email and name of referrer that will provide a PDF file separate from your application submission
or
– Submit without a Letter of Recommendation
Section IV: Essay
– 650 word essay based on one of these prompts
1) What Does “Live Free mean to you?
2) Explain what being awarded the Michelle Chalk scholarship means to you.
3) Describe your post high school dreams and ambitions. How did you come to have these goals?
4) Describe a major challenge you experienced, or difficult choice you had to make. What did you learn from the experience?
5) Describe an act of kindness you received. Have you had an opportunity to pay it forward?
Section V: Consent and Submit
– Senior Picture (Upload JPG file)
– Acknowledge and approve scholarship terms and conditions
– Submit application
Essay Presentation and Submission Tip: Write your essay using a word processor. Proof check your essay’s spelling, grammar, and word count. Do not use special formatting (bold, italic, underline, alignment, etc.). Do not indent paragraphs. Separate the paragraphs with a blank line between them.
Copy and Paste your essay into the application submission text box.
Review the application text box content to make sure your entire essay was submitted and that it appears as intended. As it appears is how it will be presented to the committees. Make any necessary adjustments. Verify the word count.
Letter of Recommendation Options: You have the option of submitting your application without a letter of recommendation, uploading a PDF file letter of recommendation, or having your letter of recommendation submitted separately to MCSF by the author.
If your letter of recommendation is coming directly from the referrer to MCSF, you will be asked to provide their name and email address. An email will be sent by MCSF to them requesting a letter of recommendation from them for you with submission instructions. Please make sure that you ask them in advance, and notify them to expect an email request from an MCSF email account.
Submitted Application: A copy of your submitted application will be sent to your school issued email address. Only one application per student is permitted.
Technical Issues?
Some tech issues may be solved by closing your browser and trying again.

Technical issues will not be an acceptable excuse for permission to submit after the cut-off.
Don’t let technical issues exclude you and your application for consideration.
